Free Dental Services for Students!

Dear Parent/Guardian,

A CPS-approved dentist will be providing dental services to students at our school in March 2024. All students in 9th-12th grade are eligible to participate, as long as the attached consent form is signed and returned to the school.  

Dental Services Offered:

- Dental Exam
- Dental Cleaning
- Fluoride Treatment
- Dental Sealants as needed
- Referral for treatment services if needed

Dental services are provided to students at no cost. If available, Medicaid will be billed, but there is no out-of-pocket cost to the student or the family. If your child has a private dentist, they may still participate in this school-based dental program. Students should receive a dental cleaning every 6 months to maintain healthy teeth; the school-based dental program allows students to receive their dental cleaning with limited loss of class time.

Please sign both boxes on the dental consent form and return them as soon as possible to your school’s main office. Your son/daughter cannot participate in the dental program unless the parent/guardian returns the consent form.
